    Download & Complete this form and bring it with you for your first apointment.

    This form will accompany any other forms you will fill out depending on the other type of services that you seek.

  • Counseling Intake Form (Form #2)

    For Counseling Appointment:

    Download & Complete this form and bring it with you for your first apointment. You will have 2 tests to take upon arrival that will take about 5-10 minutes and 1 card to complete. You will need to arrive about 10-15 minutes prior to your appointment to complete these additional forms. Thank you.

  • Adoption Inquiry Form

    Adoptive Couples/Individuals:

    Complete form and bring it in, along with Form #1 on page 1 for your first appointment.

  • Children up to age 12

    Download & Complete this form and mail to CFS before the first appointment, if you cannot mail the form please bring it with you to the first appointment. . You will have 1 test and 1 intake card to complete with your child upon arrival, that will take about 5-10 minutes . You will need to arrive about 10-15 minutes prior to your appointment to complete these additional forms. Thank you.
